AFTER SCHOOL
CLUBS — FALL 2011
The Mamaroneck Avenue
School PTA is pleased to offer a wonderful array of After School
Programs.
Please carefully review all of the instructions and save this brochure
as it contains important information and dates.
Calendar:
October 7, 2011 — Deadline for scholarship requests made to
Laura Brundage (she will be handling Dina Reynoso's work).
October 11, 2011 — Deadline for registration forms and checks.
** (No applications will be accepted after this date!)
October 17, 2011 — Fall 2011 After School Activities begin.
GENERAL INFORMATION
START AND END DATES: Individual clubs may start and end on
different dates. Please be sure to note the dates of the club sessions
carefully.
TIME: Clubs will be begin at 3:05pm and end at 4:05pm
***REFUNDS: Refunds are given only if a club has been cancelled.
Please make your selection carefully as no other refunds will be
given. Please provide a second choice when possible.******
SCHOLARSHIPS:
PTA scholarships are available for after school clubs by contacting
social worker Laura Brundage (220-3612). Please note that Laura will be handling Dina Reynoso's work. Laura will be at MAS on Tuesday mornings (1/2 day) and Fridays ONLY!
All applications
must be submitted in person and signed by a parent. Scholarships
are limited and are offered on a first come, first served basis.
Only one scholarship per child per session. Children will not be
registered for clubs until all the registration forms and all scholarship
information is completed, including payment.
The deadline for scholarship
requests is October 7th. All information will be kept confidential.
ARRIVAL AT CLUBS: Kindergarteners and 1st graders will be
escorted to clubs by our After School Club aide, Elsa Brewer. The
2nd –5th graders are responsible for getting themselves to
clubs. Attendance will be taken in clubs. If your child has attended
school but will not be attending a club on that day, please contact
the main office or your child’s teacher. Neither the PTA nor
the teacher can be responsible for a child who does not report to
an after school club.
DISMISSAL FROM CLUBS: Clubs end at 4:05 p.m. Children in
grades K-3 must be signed out by a parent, caregiver or older sibling
(grades 4-5). Children in grades 4-5 are instructed to exit through
the new building, where the After School Clubs aide, Elsa Brewer,
will dismiss them to a parent or caregiver, unless they have permission
to walk home (please check registration form).
WEATHER CANCELLATIONS/EMERGENCY PLANS: Clubs take place after
school hours and are not part of the school district’s curriculum.
If school is closed due to inclement weather or other emergency,
or if there is an early dismissal for any reason, clubs will be
cancelled for that day. If school is in session, but clubs are cancelled
due to weather or other reasons, you will be contacted. We will
attempt to make up any cancelled classes after the last session.
BEHAVIOR IN CLUBS: Please remind your child that participating
in After School Clubs is a privilege and should be fun for everyone.
Any child who compromises the safety or enjoyment of the experience
for others may be removed from the club with no refund.
REGISTRATION INFORMATION
- Registration
for clubs is on a first come, first served basis. Please submit
your application(s) as soon as possible to help ensure that there
will be room for your child in his/her first choice.

Fill out the After School Activity application by October 11, 2011
.
- Please
submit one registration form per child, per club. Additional registration
forms are available in the Main Office.
- Include
full payment by check made payable to MAS PTA for your first choice.
Write your child’s name and selected club on each check.
You must use a separate check and envelope for each child and
class. If you have more than one child and/or if you have a child
taking more than one class, you must put each individual class
application and check in a separate envelope.
- You
may wish to select a second choice on your application form. Send
checks for your first choices only! You will be notified if your
child’s first choice is cancelled or closed. At that time,
you will be given instructions for sending payment for the second
choice (which your child will automatically be enrolled in) and
the check for the first choice club will be destroyed. If you
did not indicate a second choice, you will be informed which clubs
still have openings and you can decide if you would like to choose
one of those.
- Confirm
that your child is enrolled in a club, please check the bulletin
board across from the nurse’s office 2-3 days before the
start of clubs.
- Put
the registration form and check in one envelope per club.
- On
the front of each envelope print the day of the club and the name
of the club (see example below) of your first choice.

or it will be returned to you and your child will not be registered.
