Deposit Verification Form
This form should be used whenever funds are collected on behalf of the PTA. It should be enclosed with collected funds and submitted to the PTA Treasurer.

Reimbursement/Check Request Form
This form should be used when requesting reimbursement of PTA-related expenses and/or payment of third party PTA invoices. It should be attached to receipts and/or invoices and submitted to the PTA Treasurer.

Scholarship Request Form
This form should be filled out whenever requests are made for PTA scholarships. It should be submitted, along with any relevant documentation, to the Principal.

Blue Volunteer Form
This form should be completed when you want to help with school activities.
